Internship Information Form Instructions

  1. Student needs to fill out the student information and and get the signature of the department internship specialist.
  2. Employer or company representative (arcitect for Office internship architect, architect/construction engineer for construction internship) needs to fii out the statement of the institution to do an internship, sign and stamp it.
  3. The form needs to be submitted to the department internship specialist for unit approval.
